The CFC's Policy with Regard to Protected Health Information and Confidentiality for Clients
A client of the CFC is entitled to specific rights, under state and federal HIPAA law, to have their personal health information protected. Additionally, clients can expect that information disclosed during clinical sessions will remain confidential outside of specific conditions outlined in the CFC’s confidentiality policy. Your privacy and information are extremely important to the Center for Connection (the CFC). We have created this Privacy Policy to demonstrate our firm commitment to privacy, so that you will be aware of any uses made of information that may be generated as a result of you using our website. Any information shared or provided to the CFC is not and will never be shared outside of the CFC or sold to third parties.
Information Generated when you visit the CFC Website
Depending upon your use of the CFC website, certain types of personal and non-personal information may be collected. For example, you may give us your name, the prospective client’s name, contact phone number, email address, clinical/developmental concerns, and the prospective client’s DOB when you complete an inquiry form or submit an email to the CFC. Any personal information that you provide to us will be used by the CFC to fulfill the stated objective for which the information was requested or collected (e.g. if information is provided as a result of you requesting outreach to schedule services, we will use the information provided, such as email address, to respond to you). In addition, such information may be used to update the prospective client’s information across division tracking systems in the case that the prospective client is placed on one or more waitlists for services.
Other information that may be generated and collected when you use the CFC website may be nonpersonal in nature. For example, we may collect the date and time you access the CFC website or the Internet address of the website from which you linked directly to the CFC website. We use this information to measure the number of visitors to the different sections of the website and to help make the CFC website more useful to visitors.

Contacting the CFC
Like most sites on the Internet, our website contains a feature that allows you to contact us via an inquiry form. To use this feature, you can click on the “Click Here to Get Started” button on the home page, click on the “Contact” option within the “About” drop-down menu, or click on the “Get Started” option within the “New Clients” drop-down menu. When you click any of these options, you will be directed to our inquiry form, which is received by our Director of Client Relations. In order to appropriately support you and meet your needs, you may be asked to provide certain information, such as your/the prospective client’s name, DOB, contact information, and reason for outreach. In addition to completing an inquiry form, you can contact The Director of Client Relations directly at office@thecenterforconnection.org. All emails received and sent by a CFC account are encrypted and HIPAA-compliant.
